Focus on one core idea:
A collective can contain enough knowledge and competence to avoid failure, yet still act badly because the decisive information cannot pass through the coordination bottleneck to the place where action is chosen.
The chapter’s central claim is that collective competence is not the sum of local competence. Capable parts only become a capable whole when information, authority, incentives, trust, and timing connect them to system-level action. A coordination bottleneck exists when improving one part fails to improve—or even worsens—the competence of the whole.
Main visual concept
Use a wide 2:1 composition with one immediately understandable physical metaphor.

Composition
Left: competent local parts
Use only three large source platforms.
Technical observation platform
A small team observes the deployed system through a clear instrument.
They see a visible crack, dangerous trajectory, or unstable feedback loop.
Their warning should be concrete and unambiguous:
- a bright amber signal;
- a damaged component;
- a trajectory approaching a drop.
Do not use written reports or screens full of data.
Safety and interpretation platform
A second group understands why the signal matters.
They compare the observed failure with a simple model and correctly infer that the process should stop.
Their role is interpretation, not merely observation.
Human or public-interest platform
A third small group represents those affected by the outcome.
They provide value-relevant context: what would be harmed, lost, or made irreversible.
Keep this simple—perhaps a few people near a protected garden, community, or water source.
All three groups are locally competent.
Their signals converge toward the same bridge.
Centre: the coordination bottleneck
The centre of the image should contain one dominant narrow bridge, pipe, or relay chamber.
This is the chapter’s core visual object.
Several broad signals arrive from the left, but only one thin channel exits to the right.
At the bottleneck, show three simple losses rather than all seven chapter categories.
Compression
A rich amber warning stream enters.
Only a tiny pale token or simplified signal emerges.
Important distinctions are visibly lost.
Translation
The warning passes through one conversion mechanism and changes shape.
A clear danger signal becomes a neutral administrative symbol or routine status indicator.
Do not use words or letters.
Authority separation
The final signal reaches a person or desk that can record it but cannot operate the stop lever.
A visible mechanical linkage from the warning channel ends just short of the actuator.
This is the most important detail.
The chapter repeatedly emphasizes that information can be present somewhere in the system while remaining disconnected from system-level action.
Right: the action system
On the right, show one large deployment mechanism.
Choose a simple form:
- a heavy gate opening;
- a large machine releasing a process downstream;
- or a rail carriage crossing a final threshold.
The mechanism should have:
- one prominent stop lever;
- one strong amber action path;
- one irreversible consequence just beyond it.
The stop lever is physically close to the action mechanism but far from the people who understand the risk.
A different group operates the deployment machinery.
They are not villains.
They receive:
- a weak or ambiguous signal;
- strong pressure to continue;
- an established schedule or flow.
The deployment therefore proceeds.
Irreversible consequence
Beyond the action mechanism, show one beautiful and valuable region that may be damaged:
- a living valley;
- a river;
- a community;
- or a delicate infrastructure network supporting ordinary life.
Keep it visually simple and restrained.
The purpose is to make the coordination failure matter.
Do not depict apocalypse. Show that the action is crossing a point after which correction will be much harder.
This fits the chapter’s latency problem: information arriving after irreversible action is no longer useful.
The central contrast
The illustration should clearly contrast:
Abundance on the left
- enough knowledge;
- enough expertise;
- enough warning;
- enough concern.
Scarcity in the centre
- narrow bandwidth;
- distorted translation;
- weak authority linkage.
Power on the right
- strong actuator;
- fast execution;
- high stakes;
- little remaining time.
This is the visual form of:
local competence + failed coordination = weak collective competence.
One visible alternate path
To preserve a constructive implication, include one faint but plausible improvement.
A broader amber bridge is being built beneath the bottleneck.
It directly connects:
- the technical warning;
- the interpreting group;
- the affected people;
- and the stop mechanism.
It is incomplete, but its purpose is clear.
This suggests that dashboards, safety cases, procurement clauses, audit channels, and governance structures matter when they genuinely convert complex evidence into action rather than merely producing paperwork. The chapter describes such artifacts as bandwidth-conversion devices.
Keep this secondary. The main image remains the current failure.
Visual hierarchy
The viewer should perceive, in order:
- the narrow coordination bottleneck;
- the strong warning on the left;
- the powerful action mechanism on the right;
- the broken connection to the stop lever;
- the valuable region beyond the irreversible threshold;
- the faint wider corrective bridge under construction.
Detail level
Use low detail.
Limit the image to:

Continuity with Chapter 12
Chapter 12 showed capability expansion outrunning correction.
Chapter 13 should show why adding more competent components does not automatically repair that gap:
- the technical team may understand the danger;
- the safety team may interpret it correctly;
- affected people may object;
- the deployment mechanism may still continue because those signals do not reach the actuator in usable, timely, authoritative form.
Reuse:
- blue-grey information channels;
- amber correction channels;
- one high-stakes actuator;
- one human correction chain.
But replace the expanding boundary with a single central coordination bottleneck.
Exclude
Do not include:

The failure should arise from misaligned connective structure, not from incompetent or malicious individuals.
